The Claims Bar Date of December 22, 2021 has passed. If you have missed the Claims Bar Date and believe you have a reasonable and valid excuse for doing so, please contact the Receiver through Omni Agent Solutions. In addition to submitting your Proof of Claim Form, you must provide a written explanation of the circumstances underlying your failure to timely submit your claim and why your late claim should be considered.

You have reached the claims information and submission page for the EquiAlt Receivership. Please follow the instructions below to submit your claim.

The Claims Bar Date established by the Court is December 22, 2021. Any person or entity who fails to submit a Proof of Claim to the Receiver on or before the Claims Bar Date (December 22, 2021) shall be forever barred and precluded from asserting a claim against the Receivership Entities’ assets.

A Proof of Claim may be submitted electronically on this website, or by mail, overnight delivery, or courier on or before December 22, 2021 to the following address: EquiAlt Receiver Claims Processing c/o Omni Agent Solutions 5955 De Soto Avenue, Suite 100, Woodland Hills, CA 91367.

Please Note:

You may submit multiple claims in a single log-in session, and you may log in at any time before the Claims Bar Date to submit additional and/or amended claims.

If you submit your claim online, you do not need to submit a claim by mail.

The Proof of Claim Form is fillable and may be completed on your computer for you to save and print before mailing. Alternatively, you may print the Proof of Claim Form after clicking on the link above, complete it by hand, sign and mail it to the address below. Altered forms will not be accepted.

Please Note:

Proof of claim forms may be submitted by mail, overnight delivery, or courier to Omni Agent Solutions at the above address. All Proof of Claim Forms must be sent so as to be received on or before December 22, 2021. It is the claimants’ responsibility to ensure that Proof of Claims forms are delivered to Omni Agent Solutions.

Submission of Proof of Claim Forms by email and/or facsimile transmission is not permitted.

You must submit an originally executed Proof of Claim Form.

Information Collected

The information you provide in your Proof of Claim Form will be used to determine your rights to a distribution, if any, from the Receivership. The Receiver has the right to dispute and/or verify any information you provide to determine the proper distribution amount, if any, to which you may be entitled. The Receiver further has the right to amend any information he may have provided as to your Net Investment Amount. By identifying and providing a Net Investment Amount for an investor, the Receiver does not waive any right to (1) deny, contest the validity of, or otherwise object to a claim, or (2) if warranted, amend the provided Net Investment Amount.
